Click the "Book an Appointment" button and make an appointment on the day that best fits your convenience. After that we will reach out to you via phone or email, or if you have a full service appointment, come into our location at 180 w. 15th street Edmond, OK 73013.

Once you have completed your online recommendation consultation with one of our board certified physicians, we will send you the completed required physicians form and you can upload it during your application process on the OMMA website.

If you are a full-service customer, all you need to do is wait from here on out as we take care of all of this for you.

After you have completed you full service appointment with us, or have submitted your application yourself, you just wait from an email from OMMA saying your application was submitted. After this, you should receive an approval email, or a rejection instructing you on clerical errors that need to be fixed. Don't worry, errors happen and can be quickly fixed and resubmitted!
